Next Steps Transitional Care shows you how to work out independently with the guidance of a fitness professional. You will learn how to improve your health with a regular routine of exercise and wellness in order to reduce specific risk factors and help prevent the development of certain conditions related to a sedentary lifestyle. The program is open to anyone in the community, so you do not need to be a member to participate.
